Possible causes of SAH?

One week after giving birth to our first son, my wife was admitted to the ER with 'worst headache ever'. After 18hrs she was diagnosed with SAH. After about 5 days in ICU she passed away from several strokes caused by vasospasms.
Although several scans were done throughout the week, they never found an aneurysm and never could tell me the source of the bleeding.
Is it possible that there was a ruptured aneurysm or burst vessel and they could just just find it visibly? Or does that mean that the blood did not come from the brain?
At the time of delivery, my wife was given an epidural. I remember that the anesthsiologist had to take two tries to get the needle it. The first attempt was painful for my wife, he then took the needle out and reinserted it. (no explanation was given). Is it possible that a blood vessel was pricked at that point which caused bleeding, and during the week after birth (c sec. actually), the blood traveled up the spine to her brain?
I understand that none of this can bring her back but i would like to be able (someday) to intelligently explain to my son what happened to his mom.
